Below is a list of all of the forms needed for our individual events.  All necessary forms must be completed before a student can attend an event.  The forms are in .pdf format.

Medical Permission and Release Form
A form is required for each student to attend any of our major events, and to be transported by First Baptist Church and/or any of our sponsors, chaperones, and volunteers. 

Retreat Scholarship Form
We don’t want a lack of funds to keep your student from attending an event. Complete the following application, as this helps us determine your need and allows for us to be good stewards with our limited financial resources. Typically, we can provide some form of scholarship on the event. All information is confidential and we will make every effort to, help you.
